• We will call you for payment information via Master Card, Visa, American Express or COD.
• On orders over $100, an authorized signature will be required on faxed copy of credit card sales slip.

To place an order by fax:

• Enter quantities desired.
• Enter your contact information.
• Print the form and fax it to 724-446-0988.
• We will call you for payment information via Master Card, Visa, American Express or COD.
• On orders over $100, an authorized signature will be required on faxed copy of credit card sales slip.

To place an order by phone:

• Call 1-888-778-5438 with your order and payment information via Master Card, Visa, American Express or COD.
   

A $5 Packaging and Handling Fee
is applied to all orders.
